What is the Partnership for Aboriginal Care?

The Partnership for Aboriginal Care will test a model of coordinated care for indigenous communities which builds on partnerships established between community controlled Aboriginal organisations, mainstream health service delivery and other non-government organisations.

The model is flexible, and must work across these agency boundaries, and it is strongly influenced by the existing infrastructure within each community. Coordination of care for community members will be provided from a number of Care Units. Currently, there are three Care Units, which are housed in Biripi Aboriginal Corporation Medical Centre and Durri Aboriginal Corporation Medical Service, as well as a Care Unit run from the Trial office in Port Macquarie.

The Partnership for Aboriginal Care will provide a variety of services and programs to Aboriginal community members across the Mid North Coast. The model aims to address the health and complex needs of community members, and is an important initiative in exploring opportunities for improving Aboriginal health.
